Pathology report and explantion report.
Pathology pre-op and operative report.
Package should read :
Failed medical implants not for reuse for research only. No commercial value. Nominal value $1.00.
On the courier way bill:
If the hospital sends it, it should be checked off to be billed to third party. Check off duties and taxes check off box.
If you send them then it should be billed to sender.
To prevent leakage. Hospital should put implants in a plastic container that is sealed.
